Our Mission
The Help Center is dedicated to connecting Kosciusko County residents to health and wellness providers, increasing appropriate utilization of health and wellness services and providing financial assistance to individuals who are challenged due to a medical condition.
Location
The Help Center is located at: The Health Services Pavilion, 1515 Provident Drive, Ste 130, Warsaw, IN.
Social Worker
Laura Deal-Decker, MSW meets with anyone applying for assistance or looking for information on resources available. She has been at the Help Center since November 2011. Before joining The Help Center, she was at Combined Community Services for ten years. She earned her BSW at Grace College and MSW at Andrews University. One of her favorite aspects is sharing options with people to bring hope to a difficult situation.

How do I apply?
To access assistance through The Help Center, call (574) 372-3500 for an appointment. The receptionist can schedule a time to meet with the Social Worker. You may also stop by the Health Services Pavilion to schedule an appointment. You will be asked to complete an application. You may also print and complete the application ahead of time. Download the application from application tab above.
What do you assist with?
The Good Samaritan Funds may assist directly with the cost of being seen by a physician, medical testing, medical equipment or travel to medical appointments. Our goal is for you to have access to medical care.
The Good Samaritan Fund cannot assist with large hospital bills, medical bills in collections. The goal of the fund is preventative in nature.
The Cancer Care Fund can assist with household expenses while a cancer patient is in treatment. Examples include: rent or mortgage, utilities, car payment, insurance, travel expenses, food gift certificates and gas cards.
Who is eligible for assistance?
Those who live in Kosciusko County needing medical care that cannot afford the cost may be eligible. The funds do not operate by an income scale, but do consider if the applicant has the ability to afford it themselves.
